Pre-Paid Home Phone Service

Applying
for service with Aspire Telecom® is really simple.
You can either visit one of our "Authorized Agents" or
complete and mail in the "Service Agreement" form along with your payment. Call us, Toll-Free, at
888-2ASPIRE (888-227-7473) if you need help.

Here is what is Required
to complete the form.

Print your full
name and address. Please print clearly.

The SERVICE
ADDRESS is where you want us to hook up your new phone! This
must be a PHYSICAL ADDRESS not a P.O. Box. Be very specific
when you fill out your address and be sure to include your
apartment or lot number if you have one. If you have an old
phone bill or an electric bill for service at your current
address, use the address shown on that bill.

If your BILLING
ADDRESS is different than your SERVICE ADDRESS write it in.
If not, you may leave this section blank.

You MUST include
a phone number where we can reach you or leave a message for
you - your work number or the number of a neighbor, friend
or relative
is what we're looking for here.

If you have
previously had phone service at your current address, please
write it in the space provided. Having this old phone
number can literally cut the time it takes to get your new
phone hooked up in half. If you are applying for totally new
service, then you need to provide driving directions in the
space provided on the left side of the page. Examples of
totally new service would be, a new trailer or a new house
on a stand-alone lot. In the middle of the page - The BASIC
LOCAL PHONE SERVICE Fee and INITIAL HOOK-UP/PROCESSING Fee
are for your "basic service" and are required, not
optional. For Optional Services, mark the boxes to
select those optional services you want. Leave these blank
if you don't want optional services.

Now, total up the
cost of the optional services you marked and write that
amount in the SUB-TOTAL FOR OPTIONAL SERVICES box. Put in a
zero if you did not choose any optional services. Total up
the amounts from the 1st MONTHS SUBTOTAL AND the SUB-TOTAL
FOR OPTIONAL SERVICES boxes and write that total amount in
the TOTAL FOR 1ST MONTH & OPTIONS box. Remember, the
INITIAL HOOK-UP/PROCESSING fee is a One-Time fee not a
monthly recurring fee. As long as you continue month to
month service with us you will not have to pay that fee
again.

Now SIGN and DATE
the form.

Finally mail the
form to us along with a MONEY ORDER for the total. Money
orders are the only acceptable form of payment. We do not
accept personal or company checks. Please do not send cash
through the mail.
Thats it. Once we get the signed
Service Agreement
and payment
well get right to work. Your phone service will be turned on in
(normally) 3 to 5 working
days and when its up well call you on your new line and give you your new
telephone number.
Be sure that you have a working telephone and that it is
plugged into an active telephone outlet in your home so that you can receive this
important call. |
